Registered Nurse (RN)

• Provides geriatric and/or medical case management
• Provides intermittent assessment of medical condition plan of care
• Coordinates changes in medical treatment with the physician
• Provides teaching related to disease management and medications
• Provides treatments such as wound care & I.V. infusions
• Coordinates use of tele-health equipment to monitor congestive heart failure
• Evaluates response to medications and recommends changes to physician
• Evaluates need for other services
• Coordinates overall plan of care with all team members

Licensed Practical Nurse (LPN)

• Works under the direction of the RN, who develops the plan of care
• Coordinates changes in plan with RN
• Provides teaching
• Provides treatments
